My clinical problem is

What is the purpose of the research or the proposed research question/hypothesis? The purpose of the study is to analyze ways that can be utilized in lowering the readmission rate of HF patients. The implementation of a heart failure program would create a variety of services that would in turn result in reducing and preventing readmissions thus creating safe transmissions in regard to hospital discharge. The study sought to establish whether enhanced discharge transitions leads to reduced readmissions. The purpose of the research is to establish the most effective strategy in regard to lowering the readmission rate for patients diagnosed with HF. The research was aimed at investigating whether home telemonitoring is a feasible clinical and spiritual option in lowering cardiac issues.
