Gary Johnson is a 45-year-old male who presents to your clinic with the complain

Gary Johnson is a 45-year-old male who presents to your clinic with the complaint that he is, “having a lot of heartburn.” He tells you that for the last 3-4 weeks he has intermittent pain in his upper stomach. Sometimes he feels acid in the back of his throat and his throat feels scratchy. It gets worse at night when he lays down, and sometimes he has trouble falling asleep because he is coughing. He bought some TUMS over the counter and has been taking those 1-3 times a day, depending on his symptoms. The TUMS don’t really seem to be helping and so he made a clinic appointment to see if there is anything else that might help.
Past Medical History:
Gary is generally healthy. No prior surgeries or significant medical problems.
Social History:
Gary drinks 2 cups of coffee every morning. He has a glass of wine or a beer to drink 3-4 times a week with dinner. He is active, takes daily walks and goes to the gym 2-3 times a week. Denies tobacco use.
Medications:
–    TUMS 1 tablet 1-3 times a day as needed.
Allergies: None
Vital Signs:
Weight: 80 kg, height: 5 ft 10 inches, HR: 68, RR: 14, BP: 123/82
Exam:
Physical exam is normal except slight erythema in the back of the throat.
Diagnosis:
1.    Gastroesophageal reflux disease
