Written Paper (2 page s maximum) — A single-spaced, typed report (no more than 2 pages, NOT including the reference list) must accompany the presentation and should be submitted on Blackboard. Each paper should contain the following four components.
(a) Your name/s
(b) Title of project
(c) Background information about the health disparity issue and the population/s you are addressing in your presentation.
(d) Summary of additional information that is or will be covered in your presentation. Information that pertains to your specific project type. The four (4) project types are indicated below. This section of the report could be summarized.
(e) Sources, citations, and references
o In describing the health issue and the population/s, you should obtain information from
“credible sources” as discussed in class, such as academic journal articles and government reports and periodicals. A minimum of five sources should be used for the report. You may check with me beforehand about your sources.
o Cite the sources within your narrative or your report. Use APA format. Refer to the following weblink for help. https://owl.english.purdue.edu/owl/resource/560/01/
o Provide a list of the full citations of the sources you cited in the paper. This reference list should be placed at the end of your report. The reference list is NOT included in the 2-page limit.
Application of the Social Ecological Model to a Health Disparity Issue
—Individual, Interpersonal, Organizational, Community, and Policy
Review the literature on a particular health disparity issue and indicate the population/s adversely affected.
a) Explain the health disparity issue; provide background information. Why is this a problem?
b) Describe the population that has been impacted by the health disparity issue and how.
c) Using the social ecological model as a framework, explain the factors that are associated with the problem at each level of the model. These factors should come from the literature and be relevant to your population. The sources from which you’ve identified the factors should be cited in your report and in your presentation as appropriate!
d) Indicate a solution or strategy for improvement or change at each level of the model. These strategies must come from the literature and be cited.
