The purpose of this lab: To know when and how to use an informed consent form (ICF) when conducting research that collects personally identifiable information from living adult people (“human subjects”).

Informed Consent Form (ICF)
Informed Consent Form: A document describing the nature of the research being done and the person’s involvement if they choose to become involved as a research subject. It informs them how their information may be used and if there are possible risks to being involved in the study. It also informs the subject of their right to privacy. It asks for their consent to be studied and what types of information about them to remain private (WHO 2021).

Directions
Ask a person (a friend or family member) to complete and sign an Informed Consent Form (using the PDF file) for an imaginary research project that requires you to interview them about their food preferences and dietary practices. You do not need to interview them for real because this is an imaginary reason for them to review, complete, and sign the informed consent form. Submit your work by uploading a photo (or saved PDF file) of the completed form to the Lab #0 Canvas Assignment.

–>You have two copy options for the informed consent form PDF file:
- Option 1: You can print a hard copy and then have your friend or family member read it, complete it, and sign it according to the form’s instructions for “research subject”. You need to sign it on the line for “student researcher”.
- Option 2: You can copy-paste the text of either the PDF file or the Microsoft Word file of the Informed Consent Form into an email, text, or file and have your interviewee read that, complete the form, and then sign it electronically using an application like Adobe Acrobat Pro or Chrome’s free Kami extension.
Step #3: Find a person who agrees to participate in this lab by being a pretend human subject.
- Ask them to read, complete (circle the choices on the form on what information to share or not share), and then sign your Informed Consent Form for this lab assignment. (worth 4 points)
- Both of you must sign it at the bottom: You (the “interviewer”) and your interviewee (or “subject”). (worth 4 points)
Step #4: After your pretend human subject reads, completes, and signs the Informed Consent Form, upload it to Canvas as an attached jpeg, Microsoft Word, or PDF file.
