He is alert and oriented, words are coordinated, denied suicidal ideation, verbalized skin was intact, well hydrated, no bruise, cut, and no dehydration. Client verbalized he does not have access to low scheme housing program and he remained homeless. Client expressed dissatisfaction with is living condition that has put him in a depressive state.
