Dominique was seen today via telehealth while client was in the community. She is 33yr old female client, alert and oriented X4, verbally compliant, stable, verbalized intact skin and denied pain. Client denied suicidal idea. He walks normal without assistive device. Last medical appointment was August 7th, 2023. Client has 4 children, work 50hrs a week as a home health aide and medical assistance. She is on government assistance programs that include food benefits and health insurance. Client verbalized she has anxiety and wants somebody to talk to her daily to relieve her diagnosis of anxiety. She report how she worries too much and affects everything she does daily.
