For this activity, I want you to fill out a PA form for patient Gilbert Drake. His previous Cigna insurance covered his Eliquis Rx but recently his insurance changed to Blue Shield and they are requiring a PA.
Attached you will find the progress note that you will use to fill out the PA.
Some little details to keep in mind:
The PA is Non-Urgent
Leave height, weight, pt’s authorized rep, and number blank. This was a telehealth visit.
You can be the Office Contact Person but don’t ever give them your email (or your providers, for that matter).
This is a New Therapy
For the second page you should indicate the side effects experienced by the patient due to taking Xarelto (this supports approval of the PA).
You should also state that the progress note is attached for review.
